How to Make an Outpatient Medical Claim


If your organisation has picked our Integrated Healthcare Solutions (IHS), submissions won’t be necessary as cash payment isn’t required at our panel of clinics.


For visits to non-panel clinics, specialists or government polyclinics, simply pay the charges up-front. Aon BenefitsCARE will reimburse your employees upon receiving their claims along with supporting documents.
 




How to make a Hospital and Surgical Claim

  • Download the GHS Claim Form.


  • Complete Section 1 and request your doctor to complete Section 2 (at your expense, if any).


  • Within 30 days of the treatment, visit us to submit your claim, or mail it to the address below along with supporting documents and receipts.

  • Keep a copy of the submitted documents for reference.




Tips and reminders
 

  • Claim must be submitted within 30 days along with original bills and tax invoices that show patient’s name, date of treatment and diagnosis, and the attending physician’s stamp and signature
  • Claims for specialist outpatient treatment or X-ray/laboratory test must include a copy of the referral letter by the attending physician
  • Claims for purchase of drugs must include a copy of the prescription by the attending physician

Underwriting Guidelines

Eligibility


  • Minimum group size of 2 employees
  • Singaporeans, PRs, as well as holders of Work Permit and Employment Passes

 


Participation
 

  • All employees in a company must participate in the group insurance programme (compulsory basis only)
  • All eligible employees who are in the same or equivalent position will be insured in the same plan

Takeover Policies
 

  • All pre-existing conditions will be covered for those employees who have been insured under a group medical insurance plan for a continuous period of 12 months

Occupational class
 

  • All benefits are available to Occupational Class 1 to 3 only

Application Requirements
 

  • Proposal form
  • Business Profile from the Accounting and Corporate Regulatory Authority (ACRA) website
  • List of names and identification numbers of authorised personnel who shall be signing the insurance acceptance (MAS 314)
  • Health Declaration Form if sum assured for Group Term Life and Group Critical Illness is in excess of the Non-Evidence Limit

  • Group Critical Illness Benefits - HIV/AIDS and pre-existing conditions
  • All other plans - other exclusions refer to policy
